    a community program But quickly after a good house to house survey we noticed that almost 30 of the persons with disabilities whom we found in the community could see again walk again be continent again or be prevented of disability The concept of Disability Hospital was born Mahatma Ghandi once said First they ignore you then they laugh at you then they fight you then you win It took time even to convince some of our best friends Today quite a number of Disability Hospitals are under construction in Africa and as we heard last week even in Asia plans are being made Together with our staff we can be proud that also our CBR HIV AIDS and orphan service delivery systems have served as examples and training grounds for health and disability leaders from other countries Dr Slaa and Dan whenever we needed you you have been there It was never too much You have always been understanding helpful and above all you did it with your heart We also want to sincerely thank the Government of Tanzania for being a close partner and especially for signing a 5 year agreement with CCBRT including all the benefits that normally only a government institute would receive As it looks today our collaboration may soon even intensify We have always received good support even up to the level of the President who personally awarded us our initial plot whilst court cases were mounting Recently we received a second even larger plot and we have been surprised how easy this time it has been I want to thank my dear wife and children As we all know always having a warm home to go back to is a privilege in life This job has had its difficult times as well Without their belief in my work their support and patience I would not have been able to serve the organization as I did Lastly I am very grateful to the Board and to CBM to get a successor like Mr Erwin Telemans He is the right person and ready for the job Erwin we spent a lot of time together today just one thing Keep your eyes focused on the inside of the organization the programs the people we serve the results in terms of improved quality of life whilst not underestimating the poverty trap in which they live If together with the colleagues you will be able to get that right then the outside environment will follow and be happy to follow It will be a privilege for me to stay in Tanzania and to serve as advisor to Erwin and to CBM till June next year after which the whole family will move to Belgium from where I will continue to serve the poor Dear Partners we sincerely hope that the level of support that CCBRT has enjoyed from you during my leadership will be extended to CCBRT under the directorship of Mr Telemans He is a person with

    effect as people tend not to take the supplement as consistently as needed in order to achieve the protection afforded by it We know that in Italy folic acid supplements are reimbursed by the State which is very positive as it makes supplements accessible to everybody regardless of their income The disadvantage of supplements is that taking folic acid supplements is only possible when the pregnancy is planned as they need to be taken one month before conception and during the first three months of pregnancy However we know that many pregnancies are unplanned Why is Mandatory Fortification the best option Folic acid deficiency Research shows that folic acid deficiency is common in women of childbearing age Mandatory fortification would mean that folic acid would reach all women of childbearing age before they become pregnant providing protection against NTDs for all children This would also reduce the number of associated terminations because as said before the same child is born without a disability and the IF network promotes the right to life of all children with Spina Bifida and views termination of pregnancy or active termination of life of newborns not as an option The right to live for all newborn children with impairments means the right to have access to all treatments which can improve their quality of life and prevent further impairments Disadvantage voluntary food fortification is that it is more expensive and therefore discriminatory reaching only the small part of the population who can afford to buy fortified foods In contrast mandatory flour fortification is shown to be safe and there is growing evidence for its role in preventing other congenital anomalies including cleft lip palate and heart defects Mandatory fortification means not having to rely only on public education programmes and public awareness about folic acid in other words everyone benefits regardless of social class or income Experiences in other countries USA since 1998 at the rate of 1 4 1 5 mg Canada since 1998 at the rate of 1 4 1 5 mg Chile since 2000 at the rate of 2 2 mg and most Latin American countries already have mandatory flour fortification with folic Countries that mandate that Folic Acid be added to flour have reported declines of approximately 25 to 50 in the prevalence of spina bifida In contrast limited decline in NTD rates has been observed in industrialized countries that recommend voluntary supplement use Australia New Zealand are heading for mandatory fortification The proposal has just been accepted by the FSANZ Food Standard Australia New Zealand FSANZ has notified the Australia and New Zealand Food Regulation Ministerial Council who are expected to consider this matter at their next meeting on 25 October 2006 If the Ministerial Council does not request a review the mandatory fortification standard will be gazetted shortly after with a 15 month transitional period prior to commencement No European country has yet implemented mandatory flour fortification with folic acid but it is a point of discussion in especially
